Job Summary: Review and complete individualized Plans of Care (Form 485) for recertification and resumption of care. Review clinical documentation to ensure care plans are accurate, complete, and tailored to each patient. Validate short- and long-term goals, interventions, safety measures, medications, and therapy goals. Review medication information to ensure instructions are complete and easy for patients to understand. Verify that the primary diagnosis aligns with provider documentation. Review hospital readmission risks and include appropriate interventions in the care plan. Coordinate with branch teams regarding missing information, corrections, or required updates before completing the plan of care. Place charts on hold when additional clinical information is needed to complete the review.
