Clinical Care Coordinator - Registered Nurse

The Clinical Care Coordinator is a front-line position acting as a representative of My Guardian providing the services offered by the company. The Clinical Care Coordinator duties are not limited to those listed as the position has a wide scope of duties at times dependent on the needs of the company.

Key responsibilities

Attend home assessments and create, implement and maintain client care plans, ensuring that staff are briefed and understand client

requirements

• Provide specialist nursing advice to the non-nurse Case Managers, Service Coordinators and Home Care Workers, motivating, leading and supervising staff
• Maintain up to date, clear and accurate client plans, records and meet administrative requirements in accordance with policy, procedure and funding guidelines
• Wound treatment/care and dressing changes
• Supervise the continuity of care by liaising with hospitals, other community services, doctors and allied health professionals
• Liaise with and organise allied health as required by clients

Report any changes in a client's behaviour or health/wellbeingControls, prepares, and administer treatments and medications with knowledge of purpose and adverse effects of the nursing care provided

Utilize professional assessment skills to identify physical, mental, and psychosocial needs of residents, patients and develop a care plan to meet these needs in a multi-disciplinary approach

Functions as a resource person and mentor to peer and subordinates to maintain professional nursing standards
