JOB SUMMARY Reminds and ensures patients are having proper peritoneal dialysis at home.JOB REQUIREMENTS Graduate of BS Nursing Active and unrestricted license to practice Nursing in the Philippines (PHRN) At least one year of experience in acute hospital setting

At least two years of experience preferably in Renal care, Peritoneal Dialysis or Nephrology Nursing Amenable to report at National Kidney and Transplant Institute (NKTI) at Diliman, Quezon City and within the NCRAmenable to do patient’s home visit for peritoneal dialysis checking DUTIES AND RESPONSIBILITIES Maintains an acceptable level of performance through completing the target count of tasks per day, timely processing of assigned tasks according to agreed-upon priority, consistently following process standards to ensure quality of output is up to par, adhering to company standards for schedule adherence to ensure service excellence and client satisfaction. Ensures process knowledge is up to date by participating in continuous education initiatives for existing and new process and services, maintaining an active client certification if needed, and timely completion of LMS modules as the job requires. Initiates and responds to internal and external customers' process related inquiries by using all communication applications available to ensure that the end-to-end process runs smoothly and efficiently. Collaborates with team members and actively participates in meetings, huddles, and engagement activities to achieve departmental goals and objectives. Reports work and process related issues including flagging potential problems to immediate supervisor as they become evident to ensure that they do not affect continuity of work and facilitate timely resolution of issue. Effectively communicate through inbound and outbound call through engaging in a sound conversation with the client and third-party entities, negotiating to get desired outcome to secure needed information to the client. Coordinates with patient through education on client products and coordination of schedule with the hospital be fluently conversing in English and in vernacular for patient education ensuring proper care management. Ensures compliance to company rules and regulations, which shall include but not limited to the Employee Code of Discipline, the policies on Quality Management System (QMS), Information Security Management System (ISMS), Environment Health & Safety (EHS), and Environmental Management System (EMS); Data Privacy Act (DPA); and other duties and responsibilities prescribed for all ADEC employees. Ensures compliance of all Non-Conformity Corrective Action Preventive Action Concessions (NCAPACs) and all other audit findings within the prescribed period.
